Male Factor · Toronto

Male factor contributes to roughly half of all infertility cases, but it's the half that fertility clinics historically under-investigate. The Toronto IVF clinics with strong male-factor programs do three things differently: they run a thorough semen analysis (not just a count and motility) including morphology and DNA fragmentation where indicated, they coordinate with a reproductive urologist for cases of azoospermia, severe oligospermia, or anatomical issues, and they use ICSI (intracytoplasmic sperm injection) confidently for male-factor cycles. Every clinic on this list has published andrology or male-factor experience. The clinics with deepest male-factor depth tend to have an in-house or closely-affiliated reproductive urologist; clinics without one will refer you out and add friction to an already complex workup.

Inclusion: clinic offers andrology / male-fertility services or has published treatment of male factor infertility, OR offers ICSI as a published service. Last verified May 2026.

How to pick a Toronto clinic for male factor infertility

Start with the semen analysis. A complete semen analysis includes volume, count, motility, morphology (strict Kruger criteria), and ideally vitality and DNA fragmentation testing for borderline or unexplained cases. A clinic that only quotes 'count and motility' is running an outdated analysis. Ask the clinic what's included in their standard analysis, and whether they perform morphology in-house or send it out — in-house morphology is generally more reliable.

Reproductive urology coordination is the second signal. For severe male factor — azoospermia (no sperm in ejaculate), severe oligospermia, or anatomical concerns — surgical sperm retrieval (TESE, micro-TESE, MESA) requires a reproductive urologist. A clinic with a named reproductive urology partner or in-house urologist streamlines this path significantly; a clinic without one will refer you out and add weeks to the workup.

Finally, ICSI confidence matters. For male factor IVF cycles, ICSI is the standard fertilization method — direct injection of a single sperm into each egg. Every full-service IVF clinic in Toronto offers ICSI, but the embryology team's experience with ICSI on poor-quality sperm samples varies. Higher-volume clinics see more challenging male-factor cycles and their embryologists tend to be more skilled at picking the right sperm. Ask about the clinic's ICSI fertilization rate for male-factor cycles, not their overall ICSI rate.

Questions to ask at your first consult
  • What's included in your standard semen analysis — morphology, vitality, DNA fragmentation?
  • Do you perform morphology assessment in-house or send it out?
  • Which reproductive urologist do you coordinate with for surgical sperm retrieval cases?
  • Do you offer TESE or micro-TESE in-house or through a partner?
  • What's your ICSI fertilization rate for severe male-factor cycles?
  • Do you test for sperm DNA fragmentation, and when do you recommend it?

Frequently asked questions

Which Toronto fertility clinics treat male factor infertility?

+

The four highest-volume Toronto IVF clinics — TRIO, Mount Sinai, CReATe, Hannam — have the strongest published male-factor programs, with andrology services, ICSI capability, and reproductive urology coordination. The list below filters our directory to clinics with andrology/male-factor experience.

What is ICSI and do I need it for male factor IVF?

+

ICSI (intracytoplasmic sperm injection) is a laboratory technique where an embryologist injects a single sperm directly into each egg, rather than relying on the sperm to fertilize naturally in a dish. For male factor IVF — low count, low motility, poor morphology, or after surgical sperm retrieval — ICSI is the standard fertilization method. Every Toronto IVF clinic offers ICSI; quality of ICSI on challenging samples varies with embryologist experience.

What is TESE and where can I get it in Toronto?

+

TESE (testicular sperm extraction) and micro-TESE are surgical procedures performed by a reproductive urologist to retrieve sperm directly from the testis when no sperm is found in the ejaculate (azoospermia). In Toronto, TESE is typically performed by reproductive urologists who coordinate with the IVF clinic — the procedure happens shortly before egg retrieval, and the sperm is used immediately for ICSI. Ask your IVF clinic which urologist they coordinate with and whether they offer same-day TESE-ICSI coordination.

Will OFP-funded IVF cover male factor IVF in Toronto?

+

Yes — Ontario Fertility Program funding for IVF is available for male-factor IVF on the same terms as any other patient. ICSI is bundled into the OFP cycle when clinically indicated for male factor. Surgical sperm retrieval (TESE/micro-TESE) is typically covered under OHIP as a medically necessary procedure performed by a reproductive urologist.

Should I see a urologist before going to a fertility clinic?

+

If you have an abnormal semen analysis, yes — a reproductive urologist evaluation can identify treatable causes (varicocele, hormonal imbalance, anatomical issues, prior infections) before defaulting to IVF. Some Toronto IVF clinics will arrange the urology referral as part of intake; others expect you to handle it yourself. Ask which model your clinic uses.
