PCOS fertility clinics in Toronto

Toronto and GTA IVF clinics with published experience treating polycystic ovary syndrome — protocol depth that actually matters when you have PCOS.

PCOS · Toronto

PCOS is the most common ovulatory disorder in women of reproductive age, and it's the diagnosis where clinic choice matters most. PCOS patients respond differently to ovarian stimulation than typical IVF patients — over-respond, fertilize unevenly, and carry a meaningfully higher risk of ovarian hyperstimulation syndrome (OHSS). The Toronto IVF clinics with strong PCOS programs run different protocols: GnRH antagonist cycles with careful trigger selection, lower starting doses, freeze-all strategies to minimize OHSS, and metformin co-treatment where indicated. Every clinic on this list has published experience treating PCOS. The clinics with deepest PCOS protocol depth tend to be the higher-volume centers, but smaller clinics with PCOS-focused REIs sometimes outperform on protocol individualization. Ask each clinic specifically about their typical PCOS protocol and OHSS prevention approach before deciding.

Inclusion: clinic publishes treatment of PCOS in its 'conditions treated' or sub-specialty information, or offers full IVF with ovulation-induction protocols suitable for PCOS patients. Last verified May 2026.

How to pick a Toronto IVF clinic for PCOS

PCOS-aware protocol design is the single biggest determinant of PCOS IVF outcomes. A clinic that runs the same stimulation protocol for everyone will overshoot in PCOS patients almost every time. Ask the clinic to walk you through their typical PCOS antagonist protocol: starting dose, trigger choice (GnRH agonist trigger is the OHSS-prevention gold standard for PCOS), freeze-all vs. fresh transfer policy, and how they monitor for OHSS risk through the cycle.

Metformin co-treatment is the second signal. Metformin doesn't dramatically change IVF outcomes for PCOS but reduces OHSS risk and improves cycle predictability in insulin-resistant PCOS patients. A clinic that routinely co-prescribes metformin for PCOS IVF patients is signaling sub-specialty awareness; a clinic that doesn't mention it is running PCOS cases on its general protocol.

Finally, ask about IUI before jumping to IVF. PCOS without other infertility factors often responds well to ovulation induction with letrozole plus timed intercourse or IUI — a substantially cheaper and lower-intervention path than IVF. A clinic that explores IUI first for appropriate PCOS patients before recommending IVF is practicing better medicine than a clinic that defaults to IVF for everyone who walks in.

Questions to ask at your first consult
  • What is your typical antagonist protocol for a PCOS patient with my baseline AMH?
  • Do you use a GnRH agonist trigger to prevent OHSS in PCOS cycles?
  • How often do you co-prescribe metformin for PCOS IVF patients, and when do you start it?
  • Do you typically recommend a freeze-all strategy for PCOS IVF, or fresh transfer?
  • Would you recommend IUI with letrozole before IVF for my situation?
  • How do you monitor for OHSS during my cycle, and what's your threshold for cycle cancellation?

Frequently asked questions

Which Toronto fertility clinics specialize in PCOS?

+

Most Toronto IVF clinics treat PCOS, but the highest-volume clinics — TRIO Fertility, Mount Sinai Fertility, CReATe Fertility Centre, and Hannam Fertility Centre — see the largest PCOS caseloads and have the most refined PCOS protocols. The list below filters our directory to clinics with published PCOS experience.

Do I need IVF for PCOS?

+

Not necessarily. PCOS without other infertility factors often responds well to ovulation induction with letrozole, plus timed intercourse or IUI — much cheaper and less invasive than IVF. IVF is the right path when ovulation induction has failed multiple cycles, when there are other infertility factors, or when there's a need for genetic testing. Ask your clinic whether they'd recommend IUI before IVF for your specific situation.

What is OHSS and how do PCOS-aware clinics prevent it?

+

Ovarian hyperstimulation syndrome (OHSS) is a serious complication of IVF stimulation that PCOS patients are at substantially higher risk for. PCOS-aware clinics prevent it through lower starting stimulation doses, GnRH agonist triggers (instead of hCG), freeze-all strategies that avoid the fresh-transfer hormone surge, and tight monitoring. Ask each clinic specifically about their OHSS prevention approach for PCOS patients.

Will OFP-funded IVF cover PCOS treatment in Toronto?

+

Yes — Ontario Fertility Program funding for IVF is available to PCOS patients on the same terms as any other patient: one funded cycle per lifetime, eligibility based on the standard OFP rules. The four highest-volume Toronto IVF clinics all participate in OFP and have strong PCOS programs.

Should I see a PCOS specialist or a general fertility clinic in Toronto?

+

For most PCOS patients, a general fertility clinic with strong published PCOS experience is the right starting point. Toronto doesn't have dedicated PCOS-only fertility clinics, but the higher-volume IVF clinics see enough PCOS cases that their REIs are effectively PCOS sub-specialists. If your PCOS is complex — significant insulin resistance, prior failed cycles, or co-existing endocrine issues — ask whether you'd be assigned to a specific REI with deeper PCOS experience.
